What do you wear under the visor??

I don't use the visor. Dark safety glasses with prescription inserts from work, and a "non-beard" balaclava. Of course I don't ride below -10c either.

Does a full face with heated visor still tend to fog up?

Nope.

I prefer MX helmet and beer goggles...after a while its doesn't really matter if you can see or not

Open face helmet my old driller Petey found at a garage sale. A pair of Spy's and a pair of Von Zipper goggles. Doo mountain balaclava and a thin touque. Might have to tape a pillow to my bars... Lol

ive worn mx/goggles, full face with both heated and non heated visors and imo the only way to go is an mx helmet with a good set of goggles and a haber (which i guess now is 509) humidity fan. i also could not believe the difference going from a regular mx to the carbon fiber mx, it was AMAZING, you wont be let down if your willing to spend a bit more $.

I would never wear an open faced helmet. I have broke the jaw piece off of an mx style helmet before. Don't think id be here if I was wearing an open face............ Its always "just a quick rip" that'll get ya!
