You can't run wires through the insulation of a steam pipe, they'll melt. When you're an inspector, that's top choice grade A cheese.

Even if it was cold water that's still cheesy. Pretty standard to either cut a notch out of the insulation, or make a "U" in the cable tray to go above the pipe. But considering I've seen people take an hour to make a single 90* bend in a cable tray, I can see why someone would be tempted to do this. .
